This information can help consumers make informed decisions about where to go for health care. Throughout your stay at Community Hospital, your satisfaction with all services is important. Should you or your family become concerned about the care you receive, you may contact the Quality Director at 405.602.8100 and discuss any issue that did not meet your expectations. We encourage you to voice your opinion concerning the care you have received and we welcome your observations. You may call the same number once you are discharged if you feel that you need to discuss any issues that were not discussed during hospitalization.

As part of its continued commitment to providing patients with accurate and relevant information so that they may make informed decisions regarding their care, Community Hospital wants you to be aware that:
Community Hospital is a limited liability company in which physicians possess ownership/investment interests. Therefore, Community Hospital meets the Federal definition of a physician-owned hospital as specified in 42 C.F.R. § 489.3. As a result, you may request, and we will supply to you, a list of Community Hospital physician-owners/investors.
